Part of QA PAR-457. Enable Sub-issues in disposable QA workspace, open GENE-1. Enter 'QA subtask: confirm acceptance' in New sub-issue title and press Enter. Input clears; parent still says No sub-issues yet for >30 seconds. Fresh navigation reveals persisted GENE-3. Repeat with another title: POST /issues returns 201 and creates GENE-4, but tree remains at one child (GENE-3) after completion. Expected new child row and roll-up count appear immediately after successful creation with an accessible confirmation. Users cannot distinguish success from lost work and may submit duplicates. Verify parent cache invalidation/optimistic tree update. Observed desktop and native accessibility tree, not just screenshot. Child IDs are disposable test data. Repro parent /w/design-qa-20261018/issues/GENE-1.
Implemented in the dependent QA stack: https://github.com/ParadauxIO/taskl.ie/pull/93
Combined verification passed: full API suite (1,306 tests), web lint and TypeScript, unit suite (1,211 passed; one existing skip), 483 Chromium story checks, and the production Next.js/CSS build. Desktop/mobile component evidence and a flow diagram are attached where applicable; server-rendered screenshot gaps and GitHub rollout prerequisites are explicit in the PR.
Awaiting review and merge; production application code has not been deployed by this task.
